Job Description
Hopscotch Primary Care is seeking an Advanced Practice Clinician to provide comprehensive and interdisciplinary primary care to a set panel of patients. The ideal candidate will have a minimum of three years' experience in a primary care specialty and experience with HCC documentation, ICD-10 Coding, and HRA.
Requirements
- Deliver comprehensive primary care services to a set panel of patients
- Work closely with physicians, community health workers, case managers, pharmacists, and others
- Completion of comprehensive annual wellness visits and risk assessments
