Job Description
Hopscotch Primary Care is seeking an Advanced Practice Provider to provide comprehensive primary care services to a set panel of patients, with a focus on patient outcomes, quality metrics, and patient experience. The ideal candidate has a minimum of three years' experience in a primary care specialty, with expertise in HCC documentation, ICD-10 coding, and HRA. The role involves working closely with a care team to deliver high-quality outcomes and meet high benchmarks for results on quality metrics and outcomes.
Requirements
- A minimum of three years' experience in a primary care specialty (internal medicine, geriatrics, family medicine)
- Experience with HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) Coding and HRA (Health Risk Assessments)
- An active, non-probationary state medical license
- US work authorization
