Under the supervision of the Statewide Harm Reduction Manager, performs duties relating to the HRC specific community case management and referral services as needed.
Requirements
- Provide HRC Case Management in designated county
- Conduct Community outreach, provider education and client recruitment of individuals in need of Harm Reduction Services
- Offer case management services to address obstacles while stabilizing at the HRC
- Conduct assessments to determine clients’ needs of supportive services
- Refer clients to HRC staff and/or community agency staff to assist them with psychosocial supports
- Follow-up with all referrals and clients to ensure all needs are being met
- Establish, maintain and update client case records using approved agency forms and procedures
- Prepare monthly reports in accordance with agency policies and procedures
- Maintain and update client information in required reporting system
- Represent the agency at community events, meeting and conferences
- Attend and participate in staff meetings and supervisory conferences
- Maintain working relationships with other community providers
- Observe established administrative procedures
- Assist in preparation of written and electronic reports, maintaining records and statistics of program data as required
