HRC Case Manager

Under the supervision of the Statewide Harm Reduction Manager, performs duties relating to the HRC specific community case management and referral services as needed.

Requirements

• Provide HRC Case Management in designated county
• Conduct Community outreach, provider education and client recruitment of individuals in need of Harm Reduction Services
• Offer case management services to address obstacles while stabilizing at the HRC
• Conduct assessments to determine clients’ needs of supportive services
• Refer clients to HRC staff and/or community agency staff to assist them with psychosocial supports
• Follow-up with all referrals and clients to ensure all needs are being met
• Establish, maintain and update client case records using approved agency forms and procedures
• Prepare monthly reports in accordance with agency policies and procedures
• Maintain and update client information in required reporting system
• Represent the agency at community events, meeting and conferences
• Attend and participate in staff meetings and supervisory conferences
• Maintain working relationships with other community providers
• Observe established administrative procedures
• Assist in preparation of written and electronic reports, maintaining records and statistics of program data as required
