Job Description
Deliver high-quality medical care and influence positive outcomes for patients experiencing homelessness in a recuperative care setting. Responsibilities include assessing health status, establishing care plans, promoting health literacy, and coordinating medical services.
Requirements
- Current and non-restricted LVN license in California
- 2 years of relevant experience
- High School Diploma or equivalent
- Basic computer skills
- Valid CA Driver’s License
