Job Description
IHCS provides in-home care services and is looking for a Senior Referral Coordinator to manage the referral process and ensure patients are promptly staffed with aligned providers. The role requires 3-5 years of experience in home health referral coordination, case management, or a related healthcare role, and strong knowledge of DME and/or home health referral processes.
Requirements
- 3–5 years of experience in home health referral coordination, case management, or a related healthcare role
- Strong knowledge of DME and/or home health referral processes, provider coordination, and health plan policies
- Experience with computerized medical management systems and/or web-based healthcare applications
