As an Intake and Referral Coordinator at HospiceCare in the Berkshires, you will act as the initial point of contact for new referrals or inquiries regarding HCIB Services, and coordinate referrals from initial contact through admission to services.

- Ability to maintain relationships and ongoing communication with IHC affiliate referral sources and non-affiliate SNFs in Berkshire County
- Ability to ensure accurate and timely data entry documents
- Ability to participate in the development, implementation, and evaluation of policies and procedures for continuous quality improvement
- Ability to ensure compliance with Federal, State, and local regulatory requirements and HCIB policies and procedures
