The Case Manager supports individuals receiving OPWDD services to achieve their highest level of independence. Through person-centered planning and ongoing case management, the Case Manager facilitates access to social, vocational, and community-based opportunities.
Requirements
- Valid NY State Driver's License
- Experience within a human service-related environment
- 2 years' experience working with the Special Needs Population
- Knowledge of OPWDD Policies
