Job Description
Keystone Human Services is seeking Program Coordinators to support individuals with mental health and intellectual disabilities. The role involves teaching life skills, promoting social integration, and providing direct care support. Requirements
- Support individuals in accessing community resources and building life skills
- Provide training and direct support to enhance personal competencies and promote social integration
- Deliver direct care support based on program needs
- Maintain organized record-keeping systems and collaborate with interdisciplinary team members
- Participate in on-call rotation and complete required documentation within established timeframes
