Peer Case Manager

Peer Case Manager will provide direct services to participants, offering solution-focused, short-term support or longer-term case management support, and support the design and implementation of outreach and community engagement activities.

Requirements

• Bachelor's Degree in Community Development, Sociology, or related field
• 2+ years of experience in direct service case management/social service programs
• Mental Health Services Administration Medi-Cal Peer Support Specialist Certification
• Bilingual Spanish/English
• Ability to work with low-income and vulnerable communities
• Valid CA Driver's license
• Auto insurance
