Job Description
The Medical Social Worker (MSW) provides psychosocial assessment and support to patients and families in the community receiving Home Care and Hospice services. The MSW acts as a resource specialist and provides information and referral services for practical and environmental needs.
Requirements
- Master's degree in social work from a school of social work accredited by the Council on Social Work Education
- Minimum of one-year experience in healthcare
- Excellent communication skills
- Proof of Flu vaccination
- Reliable transportation
