PRN Home Health Physical Therapist (PT)

We are seeking a Home Health Physical Therapist to join our team. As a Physical Therapist, you will provide skilled therapy services in the home setting, focusing on helping patients restore function, improve mobility, and achieve the highest level of independence possible.

Requirements

• Current, active Physical Therapist license in the State of Ohio
• 1 year of experience in home health or a healthcare facility preferred
• Current CPR certification
• Valid driver’s license, reliable transportation, and current auto insurance
• Strong clinical assessment, organizational, and communication skills
• Compassionate, patient-centered approach to care
• Ability to work independently and manage a flexible caseload and schedule
