RN Case Manager - Dunwoody/Sandy Springs/ Norcross/Doraville/Peachtree Corners

Longleaf Hospice and Palliative Care is a locally-owned healthcare provider offering end-of-life care to patients with life-limiting illnesses in the Atlanta area. The RN Case Manager is responsible for providing nursing care, identifying patient needs, and coordinating with the Interdisciplinary Team and healthcare providers.

Requirements

• Current license to practice as a registered professional nurse in Georgia
• Current and continued CPR certification
• At least one or two years of recent home care experience; hospice experience preferred
• Good verbal and written communication and problem solving skills
• Responsible, valid driver’s license; reliable, insured, automobile for making home visits required
