Job Description
RN Case Manager is responsible for providing nursing care to patients and coordinating with the Interdisciplinary Team and health care providers.
Requirements
- Current license to practice as a registered professional nurse in Georgia
- Current and continued CPR certification
- At least one or two years of recent home care experience; hospice experience preferred
- Good verbal and written communication and problem solving skills
- Responsible, valid driver’s license; reliable, insured, automobile for making home visits required
