Job Description
HomeMade, Mable, Leap, and Mable Direct are under the Attain Healthtech Group, a group focused on creating positive change for older people. The Service Coordinator role involves connecting clients with the appropriate supports and services to meet their needs, coordinating service delivery, and engaging providers. This position offers a flexible and supportive work environment.
Requirements
- Create and configure new client accounts
- Review client information, assessments, and care plans
- Coordinate the setup of in-home and clinical services
- Act as a point of contact for clients, families, and providers
- Ensure accurate booking and provider information
- Collaborate with support partners and clinical teams
