Health Navigator

As a Health Navigator, you will develop strong relationships with patients to assist them with their care, meet with patients, and educate them on their healthcare options. You will also help patients access community resources and coordinate follow-up care after hospital discharge. The role requires a self-starter who is comfortable working independently, has a strong service mentality, and is excited to tackle new challenges in a fast-paced organization.

Requirements

• Develop strong relationships with patients
• Meet with patients during the check-in process
• Call patients to schedule an office visit or remind them to pick up their medication
• Order and schedule various procedures, tests and screenings
• Identify and track progress on important gaps in patient care
