Health Navigator

Health Navigator Role Description. Develop strong relationships with patients to assist them with their care. Meet with patients during the check-in process, enter data into the Main Street software platform for reporting and tracking purposes, and call patients to schedule an office visit or remind them to pick up their medication. Assist with various patient-facing, administrative, and logistical tasks so that doctors can focus on delivering high-quality medical care to their patients.

• Enjoy meeting new people and developing relationships
• Bring a strong service mentality to your work
• Love your community and want to see it thrive
• Flexible and excited to tackle new challenges
• Love solving problems and will take whatever initiative is required to solve them
• Comfortable using data to help inform decisions and activities
• Familiar with and comfortable using multiple software platforms
• Active unencumbered driver’s license required
• GED or High School Diploma
