Health Navigator

Main Street Health is seeking a Health Navigator to work with patients and providers in rural clinics. The Health Navigator will develop relationships with patients, assist with administrative tasks, and help doctors focus on delivering high-quality medical care. The role involves meeting with patients, entering data, and coordinating care. The Health Navigator will work independently and be comfortable using multiple software platforms.

• Self-starter who is comfortable working independently
• Strong service mentality
• Ability to explain the importance of key activities that makes patients healthier
• Flexibility and excitement to tackle new challenges
• Problem-solving skills
• Comfortable using data to inform decisions
• Ability to learn and apply new information quickly
• Familiarity with multiple software platforms
• Active unencumbered driver’s license
• GED or High School Diploma
