Job Description
MaineGeneral Health is seeking a compassionate and skilled Licensed Social Worker (LSW) to join our Community Care Team. As a Care Manager, you’ll work with patients, families, healthcare providers, and community resources to improve the health and quality of life for those with the highest risk, highest cost, and highest utilization of health care services.
Requirements
- Active LSW license in the State of Maine (required)
- Previous social work experience (preferred)
- Excellent communication, teamwork, and advocacy skills
- Strong critical thinking and problem-solving abilities
- Ability to build trust and form supportive relationships with patients from diverse backgrounds
