Job Description
Mental Health America of Los Angeles (MHALA) is seeking a Case Manager for its DHS Homeless Housing and Mental Health program. The Case Manager will provide intensive case management services to clients eligible for permanent housing through the Department of Health Services (DHS) Housing for Health (HFH) program.
Requirements
- Provide case management support to high and low-acuity members
- Assist individuals at every stage of the housing stabilization process
- Outreach and engagement, including processing referrals and establishing rapport with clients
- Conduct intake and enrollment activities, assist with temporary housing, and assist with move-in to permanent housing
- Develop and implement an individualized case management services plan
- Ensure clients are linked to and accessing health, mental health, and substance use disorder services
- Assist clients with obtaining income and/or establishing benefits, and locating and securing employment and volunteer and/or educational activities
- Provide housing location services and educate clients on tenant rights and responsibilities
