Job Description
MHA provides mental health and safety net services to individuals transitioning from the NYS Correctional System or Psychiatric Hospital Systems, assisting with community reintegration. The Assisted Outpatient Treatment (AOT) Case Manager will manage patients, promote culturally equitable practices, and facilitate multidisciplinary teams to improve patient outcomes. This role involves case management, customer engagement, and contributing to AOT’s strategic goals.
Requirements
- Bachelor’s Degree in a related field (child and family studies, community mental health, counseling, education, nursing, occupational therapy, psychology, recreation/recreation therapy, rehabilitation, social work, sociology, or speech and hearing).
- NYS licensure and registration as a Registered Nurse.
- Experience with Health Homes (preferred).
- 2 years of experience (Master's degree or equivalent certification) either providing direct services or linking individuals to appropriate services.
- Exceptional written and oral communication skills.
