Job Description
Mental Health Association in Orange County, Inc. is seeking a Health Home Plus Care Manager to provide care management to individuals with mental health and chronic medical conditions. Requirements
- Bachelor’s Degree in related field (or equivalent experience)
- Two years of experience providing direct care (Master’s degree substitution for one year's experience)
- Experience with behavioral health diagnosis
- Fluency in Spanish and English
- Experience with electronic health record (EHR) systems preferred
- Valid driver’s license and driving record acceptable
