Job Description
The Case Management Engagement Specialist will engage individuals with serious and persistent mental illness, building trust and assessing their needs to create treatment plans and connect them to appropriate services. This role focuses on outreach, early intervention, and reducing barriers to care, emphasizing person-centered approaches. The position involves collaboration with schools and DCYF.
Requirements
- Bachelor’s Degree in psychology, social work, psychosocial rehabilitation or related human services field OR Associate’s degree in related field and two years’ experience working with individuals with mental illness.
- Consistent positive employment history.
- Strong EMR abilities and willingness to complete paperwork in a timely manner.
- Understanding of mental illness and desire to reduce stigma and effectively support this population.
