Clinical Case Manager - The Lodge

The Clinical Case Manager – The Lodge provides client-centered, recovery-oriented clinical case management services to adults with serious mental health needs preparing for independence. The position involves assessment, service planning, care coordination, and therapeutic interventions to promote autonomy, safety, wellness, and recovery.

Requirements

• Master’s degree in psychology, social work, counseling, psychosocial rehabilitation, or a related human services field
• Eligibility for New Hampshire clinical licensure (LCMHC, LICSW, or equivalent) and third-party reimbursement
• Experience providing community-based mental health, residential, or case management services preferred
