Job Description
First Episode Psychosis (FEP) case managers provide comprehensive case management and support for individuals experiencing their first episode of psychosis. This role involves collaborating with a multidisciplinary team, conducting assessments, crisis intervention, coordinating treatment, connecting clients with resources, and facilitating family engagement. The focus is on person-centered care and early intervention.
Requirements
- Conduct thorough assessments of clients.
- Develop and implement individualized care plans.
- Crisis intervention and de-escalation.
- Facilitate coordination of care.
- Document client interactions and progress notes.
- Stay current with evidence-based practices.
- Valid driver’s license and reliable transportation
- Successful completion of ANSA.
