Join our team as a Behavioral Health Consultant and provide integrated behavioral healthcare in a person-centered primary care home. Work closely with primary care providers to address the behavioral, physical, and psychosocial needs of our patient population.

Requirements

• Provide integrated behavioral healthcare in a person-centered primary care home
• Work closely with primary care providers to address the behavioral, physical, and psychosocial needs of our patient population
• Assess, treat, and manage patients with chronic behavioral, physical, and social conditions efficiently and effectively within an integrated behavioral health care model
• Educate patients, families, and clinical staff in techniques related to self-care
• Maintain accurate electronic health records of patients and completes documentation in timely manner
