Behavioral Health Consultant (Mobile/Street Medicine Program) - LPC, LMFT, LCSW, PsyD, PhD

Join our team as a Behavioral Health Consultant and provide comprehensive care for patients in a collaborative healthcare setting. You will work closely with primary care providers to address the behavioral, physical, and psychosocial needs of our patient population.

Requirements

• Provide integrated behavioral healthcare in a person-centered primary care home
• Act as an extension of the physician providing consultation on mental health and addiction, brief therapy intervention, referral and coordination of care, and psycho-education
• Assist in the development of patient care plans to prevent further psychological or physical deterioration
• Direct individual and crisis intervention within behavioral health care model
• Work with primary care team to coordinate specialty mental health care when needed
• Assist in execution of clinical assessments such as SBIRT
• Deliver patient care within HRSA services scope and approved locations
• Maintain accurate electronic health records of patients and completes documentation in timely manner
• Accurately codes patient charts to ensure correct billing
