Job Description
Northpointe, a Community Mental Health (CMH) organization, is seeking a passionate and driven Behavioral Health Therapist - ACT for the Dickinson, Iron, and Menominee counties in the UP of Michigan. This role involves assessment, diagnosis, treatment, and care management to individuals and groups, offering services in a community-based setting.
Requirements
- Interviews individuals to obtain information.
- Completes an initial Bio-psychosocial assessment.
- Provides linkage, referral, and ongoing case management monitoring.
- Services may include individual, group, family therapy, psychosocial assessments, mental status exams, diagnosis, and care management.
- Consults with treatment team and amends plans as recommended.
- Ensures case records follow policies.
- Conducts measurement of clinical outcome and satisfaction.
- Performs clinical peer reviews.
