Job Description
We are seeking a Community Health Worker to provide supports for clients to improve health and well-being, reduce social isolation and connect clients with long-term services in the community.
Requirements
- Honours Bachelor degree in Social Work or other relevant post-secondary education
- ASIST training
- Two to three years of experience in a community service setting
- Understanding of Social Prescribing approach to healthcare
