GILEAD - Case Manager

Gilead Community Services is seeking a Clinical Case Manager to join their multi-disciplinary team in a supervised apartment program for men. The Clinical Case Manager will provide case management, advocacy, educational, and supportive services to individuals with major mental illness.

Requirements

• Bachelor's degree in a human service field
• One year of clinical experience
• Valid driver's license and automobile insurance
• Experience with people with major mental health disorders
