GILEAD - Case Manager- Union

Gilead Community Services is seeking a Clinical Case Manager to join their multi-disciplinary supported apartment program staff. The individual will provide case management, advocacy, educational, and supportive services in a program serving adults with major mental illness and co-occurring substance use disorders.

Requirements

• Bachelor’s degree in a human service field; Master’s degree preferred
• One year of clinical experience
• Valid driver’s license and automobile insurance in good standing
• Reliable use of personal vehicle as needed
