Executive Director
5+ years of experience in long-term care administration, bachelor's degree in a related field, and a current Indiana HFA license
Certified Medication Aide/QMA
12 months of direct care as a Certified Nursing Assistant - CNA/STNA practice desired; long term care experience preferred
Lead Cook
High school diploma or equivalent, 2-5 years of cooking experience in a similar setting
Dietary Assistants
High school diploma or GED preferred, 2 years of Waitress or Waiter experience preferred, good attendance required
Maintenance Tech
High school diploma, valid driver's license, general maintenance experience, and mechanical experience
Prep Cook
High school diploma, strong customer service skills, ability to work cooperatively with others
Licensed Practical Nurses LPN
6 months of direct care nursing practice desired; long term care experience preferred
Registered Nurse (RN)
Valid Ohio RN license, at least 6 months of direct care nursing practice, long term care experience preferred
Licensed Practical Nurses LPN
Graduate of approved nursing program, valid Ohio LPN license, CPR, 6 months of direct care nursing practice desired
Executive Director
Bachelor's degree, current Ohio LNHA license, and 3+ years Long Term Care Administrator experience required
