Enhanced Care Management (ECM) Lead Care Manager - Santa Clara County

As a Lead Care Manager, you will be responsible for creating care plans and personally guiding members at every step, arranging all the services they need to thrive and building authentic, trusting relationships along the way. You will form strong, personal connections through frequent in-person visits, respond to real-life challenges, and become a pivotal support system for members.

Requirements

• Residency in Santa Clara County
• 3-5 years of experience in case management, social services, or healthcare
• Familiarity with Medi-Cal, CalAIM, and Enhanced Care Management
• Understanding of healthcare systems and local community resources
• Strong communication, empathy, and cultural competence
• Proven time management skills and attention to detail
• Competence using case management software and related tools
