Enhanced Care Management (ECM) Lead Care Manager - Santa Clara County

The Enhanced Care Management (ECM) Lead Care Manager will be responsible for coordinating care plans, arranging services, and providing advocacy for members. The role involves frequent in-person visits, comprehensive care coordination, and resource management. The ideal candidate will have experience in case management, social services, or healthcare, and will possess strong interpersonal skills, cultural competence, and empathy.

Requirements

• Residency in Santa Clara County
• 3-5 years of experience in case management, social services, or healthcare
• Familiarity with Medi-Cal, CalAIM, and Enhanced Care Management
• Understanding of healthcare systems and local community resources
• Strong communication, empathy, and cultural competence
• Proven time management skills and attention to detail
• Competence using case management software and related tools
