Enhanced Care Management (ECM) Lead Care Manager - Santa Clara County

As a Lead Case Manager, you'll create care plans, coordinate services, and advocate for members in need, forming strong, personal connections and ensuring holistic impact and compassionate care.

Requirements

• Residency in Santa Clara County
• 3-5 years experience in case management, social services, or healthcare
• Familiarity with Medi-Cal, CalAIM, and Enhanced Care Management
• Understanding of healthcare systems and local community resources
• Strong communication, empathy, and cultural competence
• Proven time management skills and attention to detail
