Job Description
Gold Star is seeking a Full Time LCSW or LMFT Case Manager to serve patients in a high-stress environment. The role involves case management, behavioral skills, crisis intervention, and collaboration with multidisciplinary teams. You'll advocate for patients, manage caseloads, and contribute to a positive patient experience. A dynamic and engaged environment is key.
Requirements
- Study patient behavior and apply behavioral, cognitive and emotional skills
- Document on shift and leave work at work
- Case coordinate with LCSW, Intake, LADAC, Psychiatrist and PA-C
- Respond appropriately to situations common among dual diagnosis and mental health disorders
- Apply Massachusetts General Collaborative Problem Solving skill building and intervention for symptomatic relief
- Maintain self-care outside of work hours
- Manage an outpatient caseload of 8-10 patients at RTC, PHP, IOP
- Manage patient crisis situations
- Act as liaison/advocate for information regarding medication changes
- Regularly follow individual and group billing procedures
- Effectively gather information to the pt and family benefit
