Job Description
PATH is seeking a Clinical Case Manager to join their Orange County Interim Housing team. This role involves providing therapeutic and case management services to residents at the Yale Navigation Center in Santa Ana, aiming to improve wellness and housing stability. The position requires a master's degree in a related field and experience in case management and health services.
Requirements
- Maintain confidential information according to HIPAA standards
- Have HMIS experience (preferred)
- Demonstrate knowledge or experience with case management techniques
- Possess high level of tolerance and understanding for individuals with urgent and multiple case management and health needs
