Clinical Support Team Provider (NP/PA)

The Petaluma Health Center is seeking a Clinical Support Team Provider (NP/PA) to provide critical treatment plan follow-up and support for patients at their sites. The role involves ensuring timely access to care, navigating EHR systems, and maintaining high standards of patient care. The position requires resident CA licensure and registration as a Nurse Practitioner or Physician Assistant.

Requirements

• Completion of a nationally accredited Nurse Practitioner or Physician Assistant Program
• Two years’ experience as a licensed NP or PA provider delivering primary care in a family practice setting, or graduation from a one-year NP residency program
• Current state of California NP or PA license
• NP and PA must hold current national board certification from an accepted certifying agency
• NP and PA must have completed a BRN recognized nurse practitioner graduate master’s degree program
• PA must have completed an accredited physician assistant master’s degree program
• Active and/or transferable DEA license to California
• BLS Certification
• Current CME as required for certification renewals
