Master's degree in Public Health or related field (preferred). 3-5 years experience in primary care, preferably in a PCMH-recognized setting. Current CA RN license and CA provider certifications required

MD/DO degree and residency completion. Board certification with current California license & DEA req. Bilingual in English & Spanish, CPR/BLS certification

Must have a valid CA medical license. Master's in PA Studies/MSN or equivalent. 3+ years clinical experience and 3-yr experience in urgent care or emergency (preference). BLS & DEA req'd

MD/DO degree, CA medical license, 3+ years clinical experience, pediatric & adult experience

Doctor of Optometry degree required. Current CA MD license with TPA/Glaucoma. Credential with MediCal preferred

PA degree req. 3+ yrs primary care experience; NP/PA certification & relevant license. PCMH enrollment preferred; BLS & DEA req. must pass credentialing exam

MD/DO degree req. Must be Board Certified in psychiatry. 3+ yrs psychiatric experience preferred. Must be able to meet credentialing/privileging requirements and have CPR certification

Doctor of Optometry degree. Experience leading an optometry practice. Experience treating complex visual/ocular disorders is required

Medical school grad + residency completion. 2+ yrs exp in health clinics preferred. Must hold state license & DEA registration

Must be a graduate of an ADA accredited dental school, possess an active dental license, complete required CE training, and have current BLS certification
