Job Description
Partners In Health (PIH) is seeking a CHW HIV/TB Supervisor for a home-based care community health worker position in Nimiyama Chiefdom, Kono District in Sierra Leone. The CHW will provide support to patients and communities, including education, psychological support, and home visits, within a large clinical team. PIH is dedicated to improving the health of marginalized populations and supporting local health systems.
Requirements
- Supervise all CHWs to ensure proper performance of job duties.
- Become familiar with patients' needs and referral to clinic staff.
- Coordinate training and ensuring adequate community-based documentation.
- Ensure sputum transport and linkage of community with facility.
- Ensure adequate community-based documentation.
- Ensure necessary equipment is available at all times.
