Job Description
The Intensive Case Manager is responsible for assessing clients with serious and persistent mental illness and co-occurring substance abuse disorders, creating Individual Case Management Plans, and providing support to help them stabilize and transition into long-term community-based housing.
Requirements
- Bachelor's degree
- 4 years' experience working with mentally ill and forensic consumers
- Valid New York State Driver's License
