RN - Nurse Coordinator

The RN Home Care Coordinator position serves as a member of the Interdisciplinary Team (IDT) under the CMS regulations, responsible for attending IDT meetings, managing provider orders, and providing phone triage to participants.

Requirements

• Graduate of an accredited Registered Nurse Program and licensed as a professional in the State of Colorado is required
• Minimum of one year of health care experience in hospital, long-term or home health care setting is preferred
• CPR and First Aid certification is preferred
• Excellent written and verbal communication skills and ability to interact clearly and effectively with internal and external customers
• Proficient in software programs, such as Microsoft office and electronic medical records
