Contributes to an organization dedicated to supporting individuals and families in creating their future by completing initial functional assessments to determine eligibility for long-term services and supports.
Requirements
- Schedules and completes all assigned assessments with the member and, if appropriate, care team in the timeframe required.
- Conducts assigned assessments in-person at the individual’s residence.
- Responsible for certifications to be submitted to the county to initiate eligibility.
- Provides information to the member and their care team about service delivery options and resources as needed.
- Maintains professional and ethical behaviors in all interactions with members, care teams, and RMHS internal staff while meeting expectations and quality standards set by RMHS.
