START Coordinator- Base Salary + ON-CALL PAY

Contribute to an organization dedicated to supporting individuals and families in creating their future. Provide ON-CALL person-centered, community-based clinical care coordination services for the START program. Develop evaluation and intervention plans for individuals with IDD and mental health needs.

Requirements

• Maintain an active caseload of individuals referred for START Services
• Complete START Coordinator certification training within 12 months of employment
• Provide outreach visits to individuals' homes, day and vocational settings regularly
• Develop Comprehensive Service Evaluations, Cross-Systems Crisis Prevention and Intervention Plans, intake/assessments, admission/discharge summaries
