Job Description
The Prevention Counselor role at the Emergency Response and Assessment Team focuses on providing support to families and children in maintaining permanency, ensuring their safety and well-being while empowering them to access resources. This is a hands-on position providing in-home services, assessing needs, and providing support in managing homes, parenting, and using community resources.
Requirements
- Respond to referrals in a timely manner.
- Conduct visits at the family's convenience (except the first visit).
- Provide appropriate level of care based on supervisor instructions.
- Complete the Family Situational Assessment.
- Assess risks and safety.
- Identify environmental factors impacting the family.
- Provide support to families and children.
- Engage with families from various backgrounds.
- Maintain relationships with families and foster trust.
- Implement the Prevention Plan.
