Job Description
The LPN/RN Intake/Supervisor Visits position involves conducting supervisory visits with clients in their homes, monitoring their health and care needs, and providing teaching and guidance to personal support aides.
Requirements
- Current Georgia license to practice nursing and render services as a RN/LPN
- Minimum of two years' experience in home health services or a related field
- Ability to supervise personal support aides
- Ability to communicate effectively
- Working knowledge of the Rules and Regulations for Private Home Care Providers
- Working knowledge of the policies and procedures of the CCSP, SOURCE, ICWP AND THE GAPP MEDICAID WAIVER PROGRAM
