We are seeking a Social Work Assistant to assist and manage a caseload of participants in a culturally-focused, technology-enabled healthcare organization. The ideal candidate will have experience working with frail and elderly populations, conducting psychosocial assessments, and providing case management services.
Requirements
- High School Diploma or Equivalency
- Associate’s Degree in Social Work, Human Services, Social Science, or relevant field preferred
- Valid State Driver’s License with a clean DMV record
- Minimum one year experience working with frail and elderly population
- Current CPR certification or the ability to obtain CPR certification
