Job Description
The Community Health Worker - Maternal Care Coordinator role serves as a central point of contact for pregnant and postpartum individuals, ensuring coordinated care across the perinatal continuum. This role focuses on improving maternal and infant health outcomes by supporting access to clinical services, addressing social determinants of health, and reducing disparities through proactive care coordination.
Requirements
- High School Diploma or Equivalent (Required)
- Valid Driver's License with an acceptable driving record (Required)
- One year of community health worker experience (preferred)
