Job Description
Sentara Rockingham Memorial Hospital is hiring a Community Health Worker- Maternal Health. The role serves as a trusted liaison between Sentara Healthcare and pregnant and postpartum individuals within the community, providing education, advocacy, emotional support, care coordination, and system navigation.
Requirements
- High School Diploma or Equivalent
- Valid Driver's License with an acceptable driving record
- One year of community health worker experience (preferred)
