Job Description
This position serves as a trusted liaison between Sentara Healthcare and pregnant and postpartum individuals within the community. The role integrates community health outreach with culturally responsive perinatal services to improve maternal and infant health outcomes, reduce disparities, and address social determinants of health.
Requirements
- High School Diploma or Equivalent (Required)
- Valid Driver's License with an acceptable driving record (Required)
- One year of community health worker experience (preferred)
