Job Description
ServiceNet is seeking a case manager for Supportive Housing in Greenfield, MA. The role involves establishing and nurturing a safe, drug-free, health-oriented environment for individuals experiencing substance use recovery. The case manager will guide clients through treatment plans, advocacy, and ensure they access relevant resources to achieve sobriety. This position requires independence and collaboration with various community providers.
Requirements
- Provide insight, support, and advocacy to clients
- Develop individual service goals with IAP creation
- Meet with clients to review treatment plan and facilitate referrals
- Complete insurance and discharge summaries
- Monitor participant progress and report to supervisors
- Respond quickly to emergencies and collaborate with providers
- Maintain accurate records and log notes
